Especially since version 2.6.2 was shuttered (tournament, Crashbot, announcers, medals, and crucially the slider throttle) the amount of overall iOS reviews mentioning cars driving off the track has increased. Some of this is because people have had legitimate problems with their sets - those often have to be worked out with customer care. Here, though, are all of the more boring reasons why you may face this issue. This won't help everyone but it's a good place to start.
Too much throttle: Unlike the "slider throttle", which would keep the cars on nearly any track, the "pedal throttle" is calibrated "hotter" for every mode of the game. In particular the weapon-free "Race" mode runs more hot than "Battle Race" to create some excitement in having to manage the throttle. My two year old can mostly keep a car on the track in "Battle Race" but not "Race" since he has a "lead foot" and overcooks it. Setting the throttle and maxing it at top speed does not work as a strategy for everything the way it used to do.
Too much throttle for too long: Closely related to (1), all vehicles now "come up to speed" like the Supertrucks always have. The curve for the supercars is steeper but it is still there. So if you just push the pedal at a certain point, your initial speed will pick up a bit from where it is in those first seconds.
Rubberband programming: The further behind you fall in the race, the faster you get, as the rubberband algorithm tries to help you catch up. This can't be turned off. If you started the race on the edge of running off, then fell behind, (2) and (3) could push you over the edge.
Pro-Speed: Check your profile to see if this setting is turned on. If it is, then your risks with the throttle are increased. Acceleration is faster and top speeds are higher. Trucks in RAGE MODE will nearly always go off track if pro-speed is enabled so be sure to turn pro-speed off if you are going to command trucks.
Hyperspeed: Anki pretty much comes out and tells you - the point of this game mode is to give you lots of excess throttle so you can mash it down long straights but then have to really pull back for the corners.
Downhill sections: The cars don't compensate for the additional forces associated with coming downhill - and the steeper the hill, the greater the risk is. Even on starter kit tracks like loopback and overpass, you can easily run off at top speeds for "Race" mode. The AI doesn't understand gradients in the track and is particularly susceptible to boosting over elevations in long straightaways.
Lane changing: Steering is not the worst it's ever been in 3.2.1, but it is still jerkier than the "old Overdrive". The cars change lanes forcefully even with small movements. If you pull for a lane change in the direction of forces pulling on the car in a turn, going into a jump, entering an intersection, or anywhere where the car isn't fully balanced, you risk a spinout or a run off. Trying to make a pass to the outside entering a turn is probably the most common here - extra points if that turn is downhill. I try not to change lanes in corners at all for this version of the game, using the straights to set up my line for the next turn and then, for an outside pass, using the momentum from the corner to allow more speed down the straightaway.
Scramble weapons and traps: Guardian's Scrambler, Guardian's Scramble Mine, Big Bang's Power Stomp, Ice Charger's EMP, MXT's Sirens, Power Zone Track Piece. If you're in an outside lane and get scrambled, especially in a 'risky' track section, watch out.
Dusty tires that don't grip - clean with tire cleaner. Rejuvenate tire cleaner by rinsing, then super-gently "scrubbing" in the tiniest drop of soap, then rinsing well, then air drying.
Slick track with cloudy grime film - rejuvenate track with clean microfiber with hand pressure more than "dusting" but less than "coarse sandpapering". You'll know it's working when the cloudiness is gone.
Humped track that's no longer flat at the edges. Try to smooth this out without getting fingerprints on the track if you can. If it's permanent, then that track piece may be RIP.
Lemon track with misaligned infrared lanes. Infrared lanes should line up tightly from piece to piece but I have found they don't always.
.. this situation usually results more in "flashing lights" and cars having to "re-find" the track than going off track but nonetheless...
"Hard" AI - the harder you make the AI, the more aggressive they drive including use of boosts. But the AI is not so smart about calibrating boosts for track condition, potential downhills, etc. If it's the AI going off track, start with the easiest commanders who drive more conservatively and that should help. If you're playing campaign against gold commanders, simplify your track.
In my opinion, the best starter kit track for keeping cars on the track when racing with the AI (or children) is QUADRA. Then WEDGE next. POINT, HOOK, and MICROLOOP are pretty good. CAPSULE is okay but the two long straightaways can be an issue. LOOPBACK and OVERPASS can create problems b/c of the downhills. As you expand your tracks, the "safest" designs are flat, have short straightaways, and have straight sections followed by curved sections, reflecting QUADRA's position as the most stable starter design.
Rails help of course but they treat the symptoms, not the disease.
